Evaluation of Response to Treatment of Small Cell Lung Cancer using Thoracic Computed Tomography.

Computed Tomography (CT) of the chest was performed as part of the initial and subsequent staging evaluations in 21 patients with small cell lung cancer who achieved a complete response on plain X-ray film after intensive chemotherapy, with or without thoracic irradiation. Patients consisted of 16 males and 5 females with a median age of 61 (range 37-75) years. Initial clinical staging revealed limited disease in 14 patients and extensive disease in 7.<BR>In all patients, CT demonstrated findings not observed on plain X-ray film after treatment. At the time of reevaluation with CT after treatment, there were four patterns: 3 (14%) cases of complete disappearance of abnormal lesion; 5 (24%) cases of partial lung collapse; 10 (48%) cases of small nodular lesion; and 3 (14%) cases of bronchial wall and pericardial thickening. The latter two patterns were associated with more frequent relapse in the chest (9/13) when compared with the other two patterns (2/8).<BR>Our study demonstrated that CT may contribute to more accurate restaging after treatment in small cell lung cancer patients and may reveal earlier recurrence in the chest.
