Pre and postoperative CEA levels were measured in 481 patients with lung cancer from 1978 to 1985 at Habikino Hospital. The CEA positive rate (≥5.1 ng/ml) was related to the cell type (positive rates for large cell carcinoma and adenocarcinoma), p-N, p-M, pathological stage and not to p-T or operative radicality.<BR>In 65 cases in which the preoperative CEA levels were more than 5.1 ng/ml, the postoperative CEA normalization rate was 72%. Prognosis was very poor in 17 cases in which CEA levels did not normalize within 2 months. Most of them died within 2 years.<BR>Postoperative elevation of CEA levels preceded recurrence in only 19%(8/22) in the preoperative normal-CEA cases. On the other hand, re-elevation of CEA levels preceded recurrence in more than half the cases (11/21), with positive preoperative CEA levels. The time lag between re-elevation and recurrence was 4 months on an average. Therefore in such cases postoperative measurement of CEA levels would appear to be useful for prediction of recurrence.
