A case of probable primary double lung cancer associated with sarcoid-like reaction.

A 74 year-old man, who had received therapy for chronic obstructive pulmonary disease, was admitted to our hospital because of a tumor shadow in right S<SUP>8</SUP> on chest X-ray films. Bronchofiberscopically a polypoid tumor was seen at the orifice of right B<SUP>6</SUP>.<BR>Right middle and lower lobectomy with mediastinal lymph node dissection was performed. Histological examination of the resected lung and lymph nodes revealed non-caseated epithelioid cell granulomas in the lung and all lymph nodes. From the a clinical. findings and laboratory data, we concluded these granulomas represented sarcoidlike reaction with lung cancer.<BR>Histologically the tumor in right S<SUP>8</SUP> was moderately differentiated squamous cell carcinoma, but the tumor in right B<SUP>6</SUP> was poorly differentiated squamous cell carcinoma. Because the two tumors were in different regions and had slightly different histology, this case was considered primary double lung cancer. However as they had similar in histology, it was difficult to say definitely whether it was double lung cancer or metastatic cancer.
