Two cases of small cell carcinoma of the lung associated with carcinomatous leptomeningitis.

Carcinomatous leptomeningitis was encountered in two (3%) out of 69 patients with small cell carcinoma of the lung treated with alternating combination chemotherapy (COMP-VAN therapy).<BR>A 54-year old male (case 1) with liver, bone and bone marrow metastases showed complete response after 3 courses of COMP-VAN therapy. He relapsed with carcinomatous leptomeningitis, as evidenced by headache, back pain and a positive spinal tap for malignant cells. He expired 7 weeks later. Autopsy revealed that small cell carcinoma cells were observed only in the meninges and in no other organs.<BR>A 64-year old male (case 2) with liver, bone and cerebral metastases, who had showed partial response after COMP-VAN therapy and whole brain irradiation, developed carcinomatous leptomeningitis. Craniospinal irradiation and intrathecal infusion of methotrexate and cytosine arabinoside resulted in a long period of the remission. He died of bronchopneumonia 9 months after onset of carcinomatous leptomeningitis.
