Diagnostic value of liver function tests in the detection of hepatic metastases in lung cancer patients.

We measured enzymatic activities of GOT, GPT, ALP, γ-GTP and LDH in 83 patients with lung cancer and examined the diagnostic value of liver function tests in the detection of hepatic metastases. The presence or absence of hepatic metastases was confirmed by postmortem examination.<BR>All enzyme levels were significantly higher in patients with hepatic metstases than in those without them. The liver function tests were of particular value in patients with small cell carcinoma.<BR>The appearance of liver dysfunction preceded the clinical diagnosis of hepatic metastases by CT scan or ultrasonography in 75% of patients with small cell carcinoma, 59% of those with adenocarcinoma, and 100% of those with squamous cell carcinoma. Therefore, liver function tests were useful for the early detection of hepatic metastases.<BR>The levels of ALP and γ-GTP were likely to increase earlier than other enzymes as hepatic metastases developed. Therefore, combined use of liver function tests, especially ALP and γ-GTP, with CT scan or ultrasonography is considered to be of importance in the early diagnosis of hepatic metastases.
