Bronchiolo-alveolar cell carcinoma was diagnosed in a 69 year-old man who complained of fever and abundant sputum. Lesions were found in the right middle and lower lobe. The preoperative laboratory data revealed marked hypoxia (PaO<SUB>2</SUB> 51.0 Torr) and hypoventilation (%VC 51%). As total resection of the affected lobes was not desirable because of low lung function, middle lobe resection was performed. The postoperative lung function tests improved strikingly (PaO<SUB>2</SUB> 82.6 Torr, %VC 61%) Subsequently, lower lobe resection was performed. The results of lung function tests after the second operation were as follows: PaO<SUB>2</SUB> 82.1 Torr, %VC 53%. Hypoxia is one of the main manifestations of bronchiolo-alveolar cell carcinoma and the resection of the lesion can improve it. Hypoxia is caused by arterio-venous shunt at the bronchiolo-alveolar cell carcinoma lesion.
