This 64-year-old man presented superior vena cava obstruction syndrome and an ill-defined widening of the upper mediastinum in chest x-ray, with a month history of shoulder pain. A cobalt 60 irradiation to the upper mediastinum rapidly improved the syndrome, but shadows of the infiltration in chest x-ray and a lot of atypical cells in peripheral blood were observed 4 months later after the onset, and he died with the course of 8 months. Enlargement of a superficial lymph node and splenomegaly had never been recognized throughout the course of his disease. We report a rare case with a retrosternal "board-like" infiltration of reticulum cell sarcoma originated in anterior upper mediastinum.
