To prevent peritoneal dialysis (PD) catheter exit site infection and to improve our CAPD program, details of catheter exit site care at leading pediatric dialysis units in Japan and North America were studied. A questionnaire was mailed to 20 dialysis units belonging to the Japan Pediatric Peritoneal Dialysis Study Group and 66 dialysis units belonging to the North American Pediatric Renal Transplant Cooperative Study (NAPRTCS) Group. The questionnaire contained questions regarding: 1. post-break in catheter care; the time of the first post operative dressing change, the procedure for exit site cleansing and the type of dressing used, 2. daily chronic catheter care; the frequency of dressing change, the procedure for exit site cleansing and catheter immobilization, type of dressing used and granting of permission for bathing or swimming.The percentage of responses to the questionnaire was 65.0% in Japan, and 43.9% in the US. The majority of post-break in catheter care in both countries involved swabbing with popidone-iodine and covering with gauze dressing, although the average time of first dressing change differed between the two countries. Dressings were less frequently changed in the US, with only 21.4% of dressings changed after 1 week. In daily chronic catheter care, cleansing with soap and water rather than disinfectant was more common in the US, whereas swabbing with popidone-iodine was used in 100% of the Japanese units. In 31% of US units, the exit sites were not covered by any dressing. Various procedures for catheter immobiliation were submitted.In conclusion, cleansing with soap and water around the exit site, and catheter immobilization are essential procedures in exit site care at peritoneal dialysis units in both Japan and US. The usefulness of disinfectant in preventing exit site infection remains to be evaluated.
