Evaluation of osteocalcin*log (INS-PTH) as an indicator of renal osteodystrophy in chronic hemodialysis patients.

This study was undertaken to investigate the value of osteocalcin (OC: ng/ml)×intact-N-specific PTH (INS-PTH: pg/ml) as an indicator of bone metabolism and bone formation in 79 hemodialysis patients. They were divided into four types according to the values of OC×log (INS-PTH): type I was smaller than 40; type II, 40-90; type III, 90-200; type IV, larger than 200. We evaluated the levels of alkaline phospatase, serum calcium, serum aluminum and β2-microglobulin among these types by the microdensitmetry method. It was considered that type II was a compensated bone metabolism group, type III a slightly elevated bone metabolism group, type IV a high bone metabolism group with osteoporosis and type I a low bone metabolism group with severe osteoporosis. These data indicate that OC×log (INS-PTH) is an indicator of bone metabolism and bone formation in hemodialysis patients.
