Development of a dialysis system using no machine (non machinery dialysis)

A non machinery dialysis (NMD) system can be applied to patients safely even in a ward where there is no dialysis machine.The NMD system consits of a small dialyzer and two bags for dialysate.For blood access, external shunt or arterial and venous canulation is used. The two bags are connected to the dialyzer. One is filled with dialysate and placed high, and the other is first kept empty and placed close to the floor. The system's negative pressure is generated by the siphon effect of the dialysate flow. There is no blood pump in the system, because the external circulation of the blood driven by cardiac output is enough for dialysis.We treated six acute renal failure patients and nine chronic renal failure patients, with a total of 131 treatments. Our conclusion is: 1) The NMD system can be applied for hemodialysis without skilled staff and in any place easily and safely. 2) All patients, including those in the ICU, who have had no sudden blood pressure drop and no disequilibrium syndrome can be so freabed. 3) As compared with CAVH, NMD is better and faster in the correction of electrolytes and acid-base metabolic equilibrium and extraction of excessive solutes, but as to ultrafiltration capability, NMD is inferior to CAVH (ultrafiltration ratio of NMD was about 100ml/hr). 4) We could apply short-trem dialysis to cardio stable patients by using a large surface dialyzer and higher dialysate flow. 5) NMD will be applicable to home dialysis and portable dialysis for chronic patients.
