<I>Objective</I>: Foreign HIV patients have no guarantee of medical care in the present situation in Japan. We summarize their actual conditions, medical problems and resolutions.<BR><I>Methods</I>: An analysis was done of twenty-three cases of foreign HIV patients at Minatomachi Medical Center during the past 10 years who had overstayed their visas. We made a follow-up survey on the basis of medical records of outpatients and inpatients those who included repatriates.<BR><I>Result</I>: By country of origin, 87% of the total cases were from South-East Asia and Sub-Saharan Africa. Their English language skills were poor and most medical facilities they visited did not have interpreters. As none of them had medical insurance because of their illegal status, their access to medical care was restricted due to medical costs and lack of medical information. The default rate among HIV-infected patients without developing AIDS was high. Most of the AIDS patients were in critical condition by their first visits to medical facilities and were suffering from opportunistic infections, which included mainly Tuberculosis and Pneumocystis carinii pneumonia. They were not able to pay the high cost of hospitalization, but some of them were able to take advantage of subsidized medical payment systems and the provisions of the Tuberculosis Prevention Law. Most of the AIDS patients died in Japan or in their home countries just after repatriation.<BR><I>Conclusion</I>: The following measures are needed to control and prevent the epidemic of HIV from spreading throughout our society, which includes foreigners who have overstayed their visas.<BR>1. To establish a system of trained medical interpreters.<BR>2. To prevent and treat opportunistic infections in accordance with the country of origin.<BR>3. To establish an international network with GOs, NGOs and medical facilities to facilitate the repatriation of patients and to exchange information on preventive and curative care in the sending and host countries.<BR>4. To provide education on prevention and to enable earlier diagnosis and treatment of both of HIV and other critical diseases, in coordination with NGOs and some foreign groups.<BR>5. To give all foreigners the right to have medical insurance to enable easy access to medical facilities and to avoid defaulting and unpaid medical costs.
