Optimum Ca concentration for the substitution fluid in hemofiltration

Comparative studies on Ca-balance were performed on the patients on maintenance hemodialysis (HD) and hemofiltration (HF) treatments. Ca concentration of the dialysate was 3.7mEq/l. Two substitution fluids with different Ca concentration (3.5mEq/l and 3.8mEq/l) were used in HF. Total serum Ca concentration increase after the treatment was less with both substitution fluids in HF group as compared with HD.Ca-balance for each treatment was then measured. Ca concentration in the substitution fluid was increased from 3.5mEq/l by 0.2mEq/l in 5 steps (3.5-, 3.7-, 3.9-, 4.1- and 4.3mEq/l). The result showed that, though there was some difference in the value among the cases, Ca-balance in the substitution fluid in general use was mostly negative. It was estimated that the positive Ca-balance almost the same as that in HD could be obtained through the use of the substitution fluid with Ca concentration of 4.0mEq/l.Accordingly, HF using the substitution fluid with Ca concentration of 40mEq/l was performed for months on the patients who had been on HF for more than 6 months. Improvement on the clinical symptoms such as bone pain, muscle pain and weakness were observed during this period. Decrease in serum PTH value and increase in Ht value were also noted.Although longer observation period is needed for the hypothesis to be more befinite, 4.0mEq/l may be considered as an optimum Ca concentration for the substitution fluid at present.
