A Case of Pigmented Contact Dermatitis Due to Machine Oil.

A 70 year-old man, a small metal factory manager, developed itchy eruptions on his face and hands in June 1989. Hyperpigment at ion was shown on the regions where he had noticed itchy sensation since January 1990. He had used a machine oil in his work-place for more than 30 years. We suspected the machine oil was a cause of his skin condition.<BR>To confirm the causative agent, we carried out patch testing with the machine oil. Open test and 20 minutes closed patch testing with the machine oil as is resulted in negative reactions. 48 hours dosed patch testing with the machine oil 0.1% pet showed an allergic reaction and eczematous change developed on the patch test region after 2 weeks. We carried out TLC analysis of the machine oil, and we could see patch test positive reaction only to the TLC fraction No.6. Control patch testing on a healthy adult revealed negative reactions to 5.0% pet of machine oil and TLC fractions 5.0% pet. We concluded that the allergen of his pigmented contact dermatitis was in the TLC fraction No.6 of the machine oil.
