Synergistic Effects of Air Embolism and Severe Hypoxia on Hemodynamics of Isolated Perfused Rat Lungs.

The synergistic effects of air embolism and severe hypoxia on pulmonary hemody-namics were investigated in isolated perfused rat lungs. I) Pulmonary arterial pressure (Ppa) increased sharply after 0.1 ml air-bubble infusion without altering either the pulmonary microvascular pressure or the venous pressure. Pulmonary arterial resistance (Ra) and total resistance (TPR) were also significantly increased by air-bubble infusion, whereas the pulmonary venous resistance was not. Indomethacin (5 μM) did not affect the increased Ra or TPR caused by air-bubble infusion. II) The Ppa increase caused by 3% oxygen hypoxia was significantly enhanced after air-bubble infusion, and the enhancement was not blocked by indomethacin. Lung weight showed a significant and transient increase by hypoxia after air infusion. These changes were not blocked by indomethacin. Pulmonary vasoconstriction and the lung weight change caused by Angiotensin II (0.3 ug) were not influenced by air-bubble infusion. It was concluded that synergism of embolism and hypoxia may induce pulmonary venoconstriction which unlikely occurs as a result of hypoxia alone, and since indomethacin had no effect on Ppa and lung weight change, cyclooxygenase appeared not to be a mediator of the enhancement of Ppa and increased lung weight during hypoxia caused by air embolism.
