Prognostic analysis of resected lung cancer using the immunohistochemical expression of PCNA and p53.

PCNA and p 53 antigens are well preserved in formalinfixed and paraffin-embedded surgical specimens and the relationship between the expression of these antigens and prognosis, using these materials, has been reported. However, using cytological materials, few such investigations have been conducted. The authors investigated the relationship between the immunohistochimical expression of PCNA (using PC-10) and p 53 (using DO-7), and postoperative prognosis using imprinted smears in 89 patients with lung cancer. Survival rates were compared among various categories according to each prognostic factor (univariate analysis) and significant factors affecting survival were determined by multivariate analysis with Cox's proportional hazard model.<BR>After observing cytological characteristics of imprinted smears with Papanicolaou staining, the specimens were destained and immunohistochemical PCNA and p 53 stainings were performed.<BR>Univariate analysis showed significant differences in survival in relation to the expression of p 53 (p<0.01) and PCNA (p<0.001). Multivariate analysis revealed curability, gender, cytological atypia and the overexpression of PCNA to be significant prognostic factors. However, no significant correlation was observed between the overexpression of p 53 and prognosis (p= 0.130).<BR>In conclusion, immunohistochemical analysis of PCNA and p 53 using imprinted smears of resected materials is useful for predicting the postoperative prognosis in patients with lung cancer. In particular, overexpression of PCNA is independently associated with shortened survival.
