<B><I>Objective.</I></B> The purpose of the current study was to evaluate the relation between the characteristics of participants in lung cancer screening, particularly with regard to age and gender, and cancer detection rate. <B><I>Methods.</I></B>Among a total of 34137 participants who underwent lung cancer screening with chest radiography and sputum cytology from April 1998 to March 2002, 31525 participants residing in eight high-population municipalities were studied. We compared the distribution of participants by age and gender and the cancer detection rate per 100000 in each municipality, and examined the correlations between the cancer detection rate per 100000 and the percentages of those aged 50 or more, those aged 60 or more, men, aged 50 or more men, aged 60 or more men.<B><I>Results.</I></B> Thirty cases (24 men, 6 women, mean age: 68.2 years) of primary lung cancer were detected and the cancer detection rate per 100000 was 95.2. Distribution of participants by age and gender was not similar in each municipality, and the cancer detection rate per 100000 ranged from 17.6 to 250.4. Statistically, there were positive correlations between the cancer detection rate per 100000 and the percentages of those aged 50 or more, those aged 60 or more, men, aged 50 or more men, aged 60 or more men. Conclusion. In lung cancer screening, the cancer detection rate has been affected by the age and gender of participants
