A 66-year-old woman was admitted with cough, hoarseness and left supraclavicular subcutaneous tumor. Tumor biopsy revealed atypical endothelial cells of proliferative capillaries stained by factor VI-related antigen, and the tumor was diagnosed as malignant hemangioendothelioma. No other site as a primary lesion was recognized other than the left supraclavicular fossa. Radiotherapy from the lower neck to the mid-mediastinum involving thyroid gland (40Gy) and the tumor (28Gy) were performed. In addition to multiple pulmonary, liver and bone metastases, left pleural and pericardial effusion that were caused by hypothyroidism due to radiotherapy appeared. She died 30 months after the onset. To the best of our knowledge, this case is the first report of primary malignant hemangioendothelioma of the left supraclavicular fossa.
