A 73-year-old man was admitted to our hospital complaining of productive cough and high fever. Chest X-ray and CT showed a progressive consolidation in the left lower lung field. Initially, pneumonia was suspected based on the chest X-ray films, but adenocarcinoma was diagnosed following cytological examination of sputum. Serum amylase level was elevated to 1, 693IU/l, the isozyme pattern of which was salivary-type. Left lower lobectomy was performed. On the first postoperative day serum amylase level decreased to within normal limits. Immunohistochemically, the tumor cells stained positively for amylase, which established the diagnosis of an ectopic amylase producing lung cancer. When signs of relapse appeared, serum amylase level increased again.
