Pulmonary Metastases of Retroperitoneal Paraganglioma 6 Years after the First Operation.

Distant metastasis of paraganglioma is rare in the literature. We report a case of bilateral multiple pulmonary metastases of retroperitoneal paraganglioma. A 69-yearold female was admitted because of abnormal chest shadow in 1992. She had a routine annual chest roentogenogram, but no abnormalities had ever been pointed out. She had undergone surgery for retroperitoneal paraganglioma in 1986. Six years after that operation, her chest roentogenogram revealed sharply demarcated multiple coin lesions ranging from 0.5 to 2.0cm in diameter in bilateral lung fields. Because neither bronchoscopic brushing examination nor biopsy of the coin lesion yielded a definitive diagnosis, exploratory open lung biopsy was performed. Left thoracotomy revealed multiple nodular tumors just below the pleural surface. Wedge resection of left S10 including an 8mm-sized tumor was performed. Excised tumor was encapsulated, elastic and hard. The surface of the tumor was homogeneously bright, yellow-pale and well circumscribed. On microscopic examination, tumor cells had relatively large nuclei with clear nucleoli and eosinophilic cytoplasm with abundant granules, and proliferated in a solid pattern with vascular stroma. These microscopic findings were the same as those of the paraganglioma operated on six years previously.
