Analysis of Surgical Treatment for Lung Cancer with Intrapulmonary Metastasis.

We examined the efficacy of a new classification produced by the American Joint Committee on Cancer (AJCC) in 1992, which defined an ipsilateral intrapulmonary metastasis (PM) as a local tumor extension instead of a distant metastasis of lung cancer. Postoperative survival time was calculated in 67 patients with lung cancer with PM. Median survival time (MST) after pulmonary resection for the patients with PM within the lobe of the primary lesion only was 25.8 months. There was a significant difference in the MST between these patients and others with PM outside the lobe of the primary lesion. Concerning the MST according to the stage of lung cancer excluding the factor of PM, the longest was that of stage I (42.9 months), while, the shortest was that of stage N (9.6 months). The MST based on the staging by the new AJCC classification of stage I and II (n=4) was 48.3 months; stage IIIA (n=21), 28.3 months; stage IIIB (n=34), 22.2 months; stage N (n=8), 11.1 months. There were significant differences in the MSTs between stage N and all other stage groups. Additionally, it showed that the prognosis of stage I and II was more favorable than stage III or N after surgical resection. Therefore, this new classification seems useful for estimating postoperative prognosis of the resected patients with lung cancer accompanied by PM.
