Argyrophilic stain for nucleolar organizer regions (AgNORs) to assess proliferative activity and malignant intensity was used in 21 cases with resected adenocarcinomas of the lung less than 2.0cm in size. The mean number of AgNOR dots per nucleus (AgNOR count) of tumor cells was 5.60, of infiltrating lymphocytes of the lung, 1.06 and of bronchial epithelial cells, 2.70. The AgNOR count of tumor cells was significantly higher than that of normal cells (p<0.01).<BR>In adenocarcinoma of the lung, the AgNOR count showed a good correlation with nuclear DNA counts. For DNA aneuploid tumors, this count was significantly higher than that of DNA diploid tumors (p<0.01). The AgNOR count increased with advanced disease, the count of nine recurrent cases (6.54±2.00) exceeded those of non-recurrent ones (4.91±1.06). In stage I, a higher Ag NOR count was noted in recurrent disease. The prognosis of cases with low AgNOR counts (less than 5) was much better than for high count cases (more than 5).<BR>These results suggested that the AgNOR count is a useful new index to evaluate prognosis and degree of malignancy in adenocarcinoma of the lung less than 2.0cm in size.
