A 70-year-old man was admitted due to hemoptysis. His chest x-ray film revealed a left hilar mass shadow and multiple nodular shadows in the left lower lobe. Histological examination of 21 nodules in the left lower lobe and the left hilar tumor after left pneumonectomy revealed squamous cell carcinoma. All of them were almost identical to the primary lesion histologically. These nodules could be grouped into 3 classes according to their size. It was suggested that these peripheral lesions were formed by repeated metastasis via the pulmonary artery adjacent to the primary lesion, and so, they were distributed almost evenly in the peripheral lung parenchyma.
