In 21 resected cases of peripherally originating lung adenocarcinoma which were less than 2cm in tumor diameter, cytomorphometrical studies were performed and analyzed in relation to their postoperative stages (p-TNM) and postsurgical courses. The atypical and pleomorphic grades of the tumor cells were scored and measured by a digital micrometer. In addition, nuclear DNA content was also measured in cytology smears. Atypia and pleomorphia correlated well with the malignancy of the tumor, and higher scores and higher DNA content were related to more advanced stage and poorer postsurgical prognosis. These results indicate that cellular atypia and nuclear DNA content are good indicators of the biological malignancy of small peripheral lung adenocarcinoma.
