A study of renal cell carcinoma in hemodialysis patients. Second report.

Ultrasonographic examination was conducted on 697 hemodialysis patients from March 1983 to March 1990. In 12 of these patients, who were diagnosed as having renal cell carcinoma (RCC), a study was performed on the relation between acquired cystic disease of the kidney (ACDK) and ultrasonographic findings.Contracted kidneys with multiple acquired cysts were found in 6 patients and contracted kidneys with few or no cysts was found in the other 6 patients. The RCC associated with ACDK showed a special and different echo pattern compared to that associated with a contracted kidney. There was a strong tendency to be multiple and bilateral in RCC associated with ACDK. It was relatively easy to detect and diagnose RCC associated with a contracted kidney. However, it was difficult to distinguish most of the RCC associated with ACDK from hematoma occurring in the cysts. If CT and angiography do not help in establishing the diagnosis, clinical follow-up is mandatory, which suggests that ultrasonographic examination would be useful in these cases.In this study, the incidence of RCC in hemodialysis patients was 1.72% (12 out of 697 patients), but it was 7.89% in ACDK (6 out of 76 patients) (p<0.05). From the above, it is important to recognize that the patients with ACDK carry the high risk of RCC when they are screened by ultrasonography.
