Endogenous ligands and the "chronicity" of renal failure.

We have investigated whether or not the accumulation of the endogenous ligands 3-carboxy-4-methyl-5-propyl-2-furanpropanoic acid (CMPF) and indoxyl sulfate (IS) in serum reflects the "chronicity" of renal failure. We have also evaluated the value of measuring serum concentrations of CMPF and IS in differentiating between non-dialysis chronic renal failure (CRF), acute on CRF (A on CRF) and acute renal failure (ARF), which is sometimes difficult clinically.Serum concentrations of CMPF and IS were well correlated with serum creatinine concentrations (Cr) in both dialyzed and non-dialysis CRF. In ARF, CMPF levels were low when compared with Cr and blood urea nitrogen (BUN), but IS levels were not necessarily lower when compared with Cr and BUN. The molar ratios of CMPF/creatinine and CMPF/urea of non-dialysis CRF and A on CRF were significantly higher than in ARF, but there were no statistically significant differences in the molar ratios of IS/creatinine and IS/urea in non-dialysis CRF, A on CRF and ARF. These results indicate that the accumulation of endogenous ligands with a strong affinity for serum albumin, such as CMPF, reflects the "chronicity" of renal failure and the measurement of serum concentrations is of clinical value in differentiating between ARF and CRF. However, the accumulation of ligands having relatively weak affinity for serum albumin, such as IS, does not necessarily reflect the "chronicity" of renal failure.
