Small-and middle-molecular-substance removal characteristics of high-performance membrane dialyzers.

Five types of dialyzers with high-performance membranes have been examined to evaluate their removal characteristics with respect to small-or middle-molecular-weight substances. Single-pass dialysis experiments were performed in an in vitro study using an aqueous solution containing urea, creatinine, vitamin B12 and inulin. Steady-state blood-inlet (Bin), blood-outlet (Bout) and dialysate-outlet (Dout) concentrations of these solutes were determined. Solute-removal characteristics were evaluated using an overall mass-transfer coefficient (K0), a characterization parameter for solute removal. The dialyzers used in this study consisted of an FB-190UGA made of cellulose triacetate (CTA) and FB-210H made of cellulose diacetate (CDA) (Nipro Co., Ltd., Japan), a BK-1.6P and 2.1P made of polymethylmethacrylate (PMMA) (Toray Industries, Inc., Japan), and a KF-201C-1800 made of ethylene vinylalcohol (EVAL) (Kuraray Co. Ltd., Japan).Six patients with chronic renal failure were involved in an in vivo study. Blood and dialysate samples were collected 1, 2, and 3hr after the start of treatment with filtrate flow rates (QF) of 0, 10, 20 ml/min respectively.In these experiments, FB-190UGA yielded a larger KA/A0 value for urea than any of the other dialyzers. The symbols A and A0 represent effective and catalogue membrane surface area, respectively. A similar tendency was shown in the case of middle molecules such as vitamin B12 and inulin. KA/A0 values for BK-1.6P obtained during an in vitro study, on the other hand, were 37.4×10-3 and 19.6×10-3cm/min for urea and creatinine, respectively, i.e., considerably lower than in vitro.
