Factor VIII clotting antigen (VIIICAg) and VIIICAg-phospholipid (PL) complexes in standard or activated prothrombin complex concentrates were determined using a two-site solid phase immunoradiometric assay (IRMA) for VIIICAg.All of the 3 commercial PCCs (Proplex, PPSB, Konyne) and the 2 APCCs (Autoplex, FEIBA) contained measurable amount of VIIICAg (6.2-389U/dl). Then, the action of phospholipase-C (PL-C) on VIIICAg-PL complexes in normal plasma, cryoprecipitate, factor VIII concentrate or (A) PCC was investigated, with enzyme activity monitored by thin layer chromatography. Treatment of factor VIII fractions with PL-C (5u/u VIIICAg) resulted in 25-30% increases in VIII fractions, while (A)PCC showed 25-80% increase, indicating VIIICAg-PL complex present to a greater extent than in factor VIII fractions. The presence of VIIICAg-PL complexes in (A)PCC may thus lead to the protection of VIIIC from inactivation by antibodies and could have important clinical application in treatment of hemophiliasc with inhibitor.
