ERYTHROPOIETIN FOR CARDIAC SURGERY WITHOUT HOMOLOGOUS BLOOD TRANSFUSION

Erythropoietin was used preoperatively in three cardiac surgery cases. Operative procedures were coronary artery bypass grafting in two cases, and valve replacement in one.<BR>The reticulocyte count and blood hemoglobin level had begun to increase a week after administration of erythropoietin, while autologous blood donation was also performed before the operation in two cases. No homologous blood transfusions were needed because of the increased hemoglobin level or autologous blood deposit. No adverse effects were recognized in any of the three cases and all were discharged without complications.<BR>It was concluded that erythropoietin administration combined with autologous blood donation in advance of the operation offers an excellent means of preventing homologous blood transfusion. Further study concerning the appropriate application of erythropoietin seems to be required.
