In order to assess the validity of its application in the early detection of small airway abnormalities, we investigated transit time analysis of the forced expiratory volume (FEV) curve in 154 normal subjects.Dividing the forced vital capacity into units of air and assigning a transit time to each unit, FEV curve can be described as a distribution of transit times. The degree of elongation of this time required for the air unit to be expired from the lung (transit time) is expressed in terms of a stochastic method or moment analysis. This method is theoretically superior to the conventional indices (eg. FEV1.0%, V25/HT or V10/HT) for evaluation of small airway function in that the whole data of the curve is utilized, and that the later part of the curve is especially emphasized. The mean transit time (MTT) estimated in the subjects in this study was 0.690sec (mean) and variance of transit times was 1.156sec2 (mean). These parameters significantly correlated with conventional indices. The MTT were notably different with advancing age and history of smoking. Furthermore, the magnitude of dispersion of transit times (eg. coefficient of variance) can also be derived as a potential indicator of the ventilatory inhomogeneity of the lung. It is suggested that the moment analysis of the transitime can provide a new basis for the investigation of the FEV curve.
