Decreased Binding of WGA in Epithelial Cells of Oral Lichen Planus

The purpose of this study was to investigate he distribution of wheat germ agglutinin (WGA) binding sites on the buccal epithelium of oral lichen planus (OLP). The sections of tissues obtained from 5 OLP patients and 4 volunteers (control cases) were labeled with FITC-WGA for confocal laser scanning microscopy (CLSM) and gold conjugated WGA for transmission electron microscopy.In control cases, the staining with FITC-WGA was seen at cell surface and Golgi apparatus of the epithelial cells. The staining of cell surface was more intense in spinous cells than in basal cells. The cell surface and Golgi apparatus of spinous cells in OLP cases were stained less intensely than those in control cases and showed a similar staining intensity to that of basal cells in OLP cases.The stereological analysis of ultrastructure and WGA-binding sites revealed that peak labeling of desmosome was on the 5th layer of epithelium of control cases, and the labeling was significantly less in OLP cases than in control cases. WGA-binding on the cell membrane and the number of desmosomes per cell were not different between the two groups. However, since the circumferential length of cell membrane was significantly larger in OLP cases than in control cases, the number of desmosomes per unit length of cell membrane was smaller in OLP cases than in control cases.The results suggested that decreased WGA-binding on the desmosome and/or cell surface in OLP may be a result of decreased glycosilation in the Golgi apparatus, and this compositional change of desmosome as well as the decreased number of desmosomes per unit length of cell membrane may lead to the change of cell-to-cell adhesion in OLP.
