Intractable aspiration is a severe and often fatal problem in patients with impaired protective function of the larynx. This problem is usually a result of central nervous system disorders such as a cerebrovascular accident. Surgical separation of the upper respiratory tract from the digestive tract can prevent recurrent contamination of the respiratory system in these patients.<BR>From 1988 to 1991, we performed 8 laryngectomies for senile men who had been tracheostomized for more than one year due to complications of the cerebrovascular disorders. In these cases, laryngectomy was not avoidable to prevent bleeding from the trachea and/or aspirations with the overflowing saliva. The narrow-field laryngectomy or total laryngectomy was an excellent procedure for the definitive separation of the upper digestive and respiratory systems to prevent aspiration, resulting in the improvement of quality of life for the patients and their attendants.
