Effectiveness of the Two-step Diabetes Education Program.

whether this disease could be satisfactorily controlled for a long period by an outpatient clinic.All subjects attended the basic diabetes education course. The patients with particular problems (including obesity and complications) were selected, classified into insulin therapy, obesity and diabetic nephropathy groups, and given advanced diabetes education. Details about the patient education were clarified, and the therapeutic effects of the nutrition program were assessed by using hemoglobin A1C (HbA1C) as an index.Those patients completing the basic course had a significantly lower HbA1C level after 6, 12, 18, 24, 30 and 36 months.Twenty-six patients received advanced diabetes education 1 year after completing the basic course (6, 8 and 12 were respectively in the insulin therapy, obesity, and diabetic nephropathy groups). The HbA1C level was higher for 2 years after starting the education program in the patients attending the advanced course than in those attending only the basic course, but was lower in the former group after 3 years.The two-step program proved useful for maintaining good long-term glycemic control. It provided initial patient education soon after diabetes diagnosis and advanced education 1 year after completing the basic course for those patients selected according to their problems.
