Treatment of lung cancer associated with aortic aneurysm.

In 65 cases of aortic aneurysm which we treated from 1985 to 1990 a coexisting lung cancer was encountered in 3 instances (4.6%). Retrospectively, the simultaneous occurrence of aortic aneurysm and lung cancer was observed in 2 cases. One of two patients with abdominal aortic aneurysm and lung cancer had aneurysmectomy first because the aneurysm was about 7 cm in transverse diameter ; examination of the lung tumor was refused by the patient untill two years later, when it was resected. This patient is alive without recurrence fortunately. The other had pulmonary resection first and aneurysmectomy 5 months later. The aneurysm was 5 cm in diameter ; this patient is also alive and well. The third patient was found to have lung cancer with multiple metastases 5 years after surgery for aneurysm of the descending thoracic aorta. He died of lung cancer soon after the diagonsis.<BR>In cases of coexisting lung cancer and aortic aneurysm, the assessment or treatment of lung cancer tends to be late. In regard to the priority of treatment, the size of the aneurysm and the stage of the lung cancer should be considered. If the transverse diameter of the aneurysm is larger than 7 cm, there is a high risk of rupture, so surgery for the aneurysm preceds pulmonary resection. It is desirable to avoid concomitant operation of lung cancer and aneurysm.
