Anterior mediastinal tracheostomy with pectoralis major musculocutaneous flap for primary tracheal carcinoma; a case report.

We treated a patient with primary tracheal carcinoma with laryngotracheal resection, anterior mediastinal tracheostomy and a pectoralis, major musculocutaneous (MC) flap. The patient was a 50-year-old man complaining of bloodysputum. Bronchofiberscopy revealed the tumor, about 7 cm in length, in the middle portion of the trachea extending 3 cm above the carina. Radiotherapy and chemotherapy were not effective.<BR>Laryngotracheal resection was performed, leaving a 2 cm tracheal stump, through a median sternotomy. The mediastinal tracheal stoma was covered with pectoralis major MC flap. The postoperative course was uneventful. This experience loads us to advise the pectoralis major MC flap as a simple and reliable technique after mediastinal tracheostomy, because it has not only exellent vascularity and mobility but also provides bulk for the obliteration of dead space.
