Pleuropneumonectomy with radical thymectomy for invasive thymoma with pleural dissemination.

A 50-year-old woman was admitted with massive left pleural effusion. A specimen obtained by percutaneous pleural biopsy showed thymoma. Chemotherapy with CDDP, doxorubicin hydrochloride and prednisolone and radiotherapy of the mediastinum and left pleural cavity were effective. Temporarily the left pleural effusion was reduced and the shifted mediastinum returned to its normal position, but left pleural effusion increased again, and repeatchemotherapy was not effective. Radical thymectomy and left pleuropneumonectomy were performed under the general anesthesia. Tumor cells were dissminated in the left pleural space and had invaded the left lung. Post-operatively no serious complications were observed. She is doing well 1 year and 4 months after the operation.
