Epidural fentanyl infusion for pain relief after thoracotomy.

From November, 1989, to April, 1990, 30 patients undergoing thoracotomy were given 25 μg /hr of epidural fentanyl continuously for relief of postoperative pain and a bolus infusion of 100 μg as a booster at the time of chest physiotherapy. All the patients were relieved of pain by continuous infusion when they were resting. Booster infusions of 100 μg of fentanyl reduced the pain associated with coughing. The pulmonary function tests during the procedure showed significant improvement of FEV 1.0, FVC, and PEFR. No side effects were noted except for mild itching in three patient. The combination of continuous and bolus epidural fentanyl infusion is an effective and safe analgesia for post-thoracotomy pain.
