Malignant chest wall tumor involving thoracic vertebrae resected en bloc-a case report

A 33-years-old man had had a chest wall tumor resected 20 years ago. This time, he had back pain and a chest X-ray film showed recurrence of the chest wall tumor. CT and MRI showed that the tumor involved the erector spinae muscle, thoracic vertebral bodies and spinal canal. The operative procedure was en bloc resection of the tumor with the involved lung, muscle and the left half of vertebral bodies Th 6 to Th 9. The preoperative CT and MRI images made it possible to plan the resection plane of the vertebrae to give enough surgical margin. Along the designed plane, the vertebrae were transected with osteotomes, and en bloc resction could be done without exposing the tumor to the cut surface. The pathological specimen was suspected to be localized malignant mesothelioma of pleura. By means of vertebral body resection and arch resection techniques, en bloc resection is possible for malignant chest wall tumors or lung tumors involving the vertebrae.
