Two cases of bronchogenic cyst in the posterior mediastinum

Two patients were treated surgically for bronchogenic cyst in the posterior mediastinum. One was a 47 year-old male with an abnormal mass in the posterior mediastinum on chest X-ray, CT and MRI. A neurogenic tumor was suspected, but at thoracotomy a cystic tumor, 3. 5 cm in diameter was found adjacent to the descending aorta. There was no communication with a bronchus or the esophagus and no feeding artery or draining vein. Pathological examination showed a cystic lesion lined with bronchial epithelium covering bronchial glands and cartilage and a solid lesion composed of bronchio-alveolar tissue. Although it was difficult to differentiate bronchogenic cyst from a typical extralobar pulmonary sequstration, the main lesion was cystic on macroscopic examination, so a diagnosis of bronchogenic cyst was made.<BR>The other patient, a 23 year-old male, had a tumor located near the aortic arch with no connection with a bronchus or the esophagus. Microscopic examination showed columunar epithelial and cartilage covered by the internal wall of the cyst, so a diagnosis of bronchogenic cyst was established.<BR>It is recently been suggested that bronchogenic cyst and pulmonary sequestration may share a common embryologic origin, so "congenital broncho-pulmonary foregut malformation" or "congenital cystic disease of the lung" is proposed as the proper term for these anomalies, but the details of classification remain inconsistent and confusing. Further studies are needed to clarify the nature of there letios.
