Results of curative resection of pN2 non-small cell lung cancer.

This report analyzes the results in 78 patients with N2 non-small cell lung cancer treated with complete resection and complete mediastinal lymph node dissection (N2 status was determined postoperatively). There were 44 adenocarcinomas, 25 squamous cell carcinomas and 8 others. The overall 5-year survival rate was 19.0 %. No significant difference in survival was found between adenocarcinoma and squamous cell carcinoma patients. The survival of pTl patients was significantly better (p < 0.05) than pT2, that of pT3 or pT4 patients. The survival of cN0 or cN0 + N1 patients was significantly better (p <0.01) than that of cN2 patients. The survival of those with less than 5 metastatic mediastinal lymph nodes significantly better (p <0.05) than that of those with 5 or more metastatic lymph nodes. The greatest frequency of metastases at each mediastinal lymph node level from tumors in each lobe were as follows : the superior mediastinal level for right upper lobe tumor, the inferior mediastinal level for right middle and lower lobe tumors, the para-aortic level for left upper lobe tumors, and the inferior mediastinal level for left lower lobe tumors. In all pN2 patients who survived longer than 3 years, the mediastinal lymph node metastases were limited to the above mentioned levels of greatest frequency for each lobe and the number of metastatic lymph nodes was lower than 5.
