Preoperative chemotherapy for advanced non-small cell lung cancer; a preliminary study

Preoperative chemotherapy was administered to 16 patients with advanced non-small cell lung cancer; adenocarcinoma (N =11), large cell carcinoma (N =3), squamous cell carcinoma (N =1) and carcinoid (N =1). The pathological stages were I + II (N =3), IIIa (N =7), IIIb (N =1) and IV (N =5). The reasons for preoperative chemotherapy were mediastinal node involvement (N =9), mediastinal invasion of primary tumor (N =2), metastasis (N =2), and others (N =4). Clinical responses to chemotherapy included one complete and four partial responses. Improvement in the clinical stage was observed in five patients, but, improvement in the pathological stage was observed in only two. The patients who underwent complete resection of the tumor and regional lymph nodes did better than those who had comparable stages of carcinoma who underwent incomplete resection. The problems which will be encountered in future controlled trails as well as the problems of feasibility studies are discussed.
