Interstitial Hyperthermia for Cancer of the Tongue using an Implant Heating System

Several papers have already been reported on the clinical use of hyperthermia to treat oral cancer. However, many of them were dealing with the treatment of cervical lymph node metastasis. Few reports of hyperthermia to treat primary lesion of oral cavity were found becouse of difficulty in applying hyperthermia. We are introducing and reporting the results of interstitial hyperthermia of cancer of the tongue using Implant Heating System (IHS) combined with other treatments.<BR>Six cases of cancer of the tongue has been treated. All cases were pathologically diagnosed as squamous cell carcinoma. Among 3 inoperable cases, the first case was treated by IHS (10 times), irradiation (60 Gy) and chemotherapy (CDDP : 200mg, PEP : 80mg), second case was treated by IHS (5 times), irradiation (23 Gy) and chemotherapy (5-FU : 2500mg) and the third case was treated by IHS (10 times) and chemotherapy (PEP : 80 mg, 5-FU : 2500mg). The other 3 cases were treated by hyperthermo -chemotherapy as a pre-operative therapy. These cases were treated by IHS (7 times) and chemotherapy (CDDP : 200mg, PEP : 70 mg).<BR>An IHS consists of three parts : ferromagnetic implant needle with low Curie point (Tc), induction coil and generator of high frequency magnetic field. The generator has 2.5 kw of maximum power, yielding the high frequency of 240 kHz. Ferro-magnetic implant (Fe-Pt alloy) is 1.8 mm in diameter and 15, 20, 25 mm in length, of which Curie point is set at 68°C. Hyperthermia was made using this system at 43°C for 45 minutes once or twice a week.<BR>The response were evaluated by clinical and pathological findings. The clinical response were 3 CR and 3 PR. The pathological findings in 3 cases of pre-operative hyperthermo-chemotherapy showed complete response. All cases are free of recurrence until now. Our results may indicate that hyperthermia using IHS combined with chemotherapy or radiation therapy is effective in the treatment of the oral cancer. It is expecting that this combination therapy will be a new mode of treatment for oral cancer.
