Localized Thermochemotherapy of Solid Tumors:Analysis of Joint Research at Multi-institutions

Localized thermochemotherapy was applied to the patients of large, deep-seated tumors at the 8 institutions which belonged to Matsuda research team under a Grant-in-Aid from the Ministry of Health and Welfare of Japan. We have studied tumor response and side effects, and discussed significance and problems of this therapy.<BR>1. We used Thermotron RF -8 equipment for 57 cases at 5 institutions, BSD -1000 equipment for 56 cases at 3 institutions, and other type of heating equipment for 9 cases. We administered 5 FU for 84 cases, MMC for 64 cases, CDDP for 30 cases, ADM for 17 cases, and other several kinds of drugs.<BR>2. The number of heating sessions ranged as follows : Four sessions or less for 34 cases, 4 to 6 sessions for 23 cases, 7 to 9 sessions for 19 cases, 10 to 12 sessions for 25 cases, 13 sessions or more for 21 cases. The temperatures within tumors ranged as follows in terms of degrees in Celsius : Forty or lower for 4 cases, 40s for 7 cases, 41s for 35 cases, 42s for 33 cases, and 43 or higher for 42 cases.<BR>3, The treated organs included each 28 cases of the colorectal cancer and stomach cancer, 16 cases of pancreatic Cancer, 14 cases of lung cancer, 12 cases of breast cancer, 7 cases of hepatocellular cancer, 6 cases of sarcoma, and 11 cases of others, totaling to 122 cases. The abdominal tumors occupied 75%, and 73% of the tumors were larger than 5 cm.<BR>4 Evaluation of treatment effectiveness was possible for 101 cases. The tumor response was as follows : CR for 7 cases, PR for 23 cases, MR for 19 cases, NC for 43 cases, and PD for 9 cases. The effectiveness ratio was expressed as percentage of CR plus PR to the total number of cases. It was 30%. The tumor response did not correlate to the size of tumors and the number of heating sessions. The higer temperatures in the tumor were more effective for treatment. As for improvement of clinical conditions, cancerous pain was mitigated or disappeared in 35 cases (68.6%) out of 55, and ps was improved in 32 cases (39%) out of 80.<BR>5, The side effects accompanying hyperthermia were as follows : As major local symptoms, pain, sensation of heat and burns were noticed in 25.4%, 13.9%, and 10.7%, respectively. The systemic symptoms accompanied chemotherapy, but it was not enhanced by combination of chemotherapy and hyperthermia.
