The Effects of Hyperthermia in Relief of Cancer Pain.

Pain is one of the most serious causes deteriorating the performance status (PS) or quality of life (QOL) of the patients with cancer. Between April 1986 and December 1993, 147 patients with advanced or recurrent cancers were treated by combination therapy of irradiation and hyperthermia at UOEH hospital. There were 13 patients with Pancoast tumor and 15 with recurrent rectal or rectosigmoid cancer. We evaluated the effectiveness of combination therapy in controlling pain and increasing QOL in those patients. They were all judged to be inoperable and complained of severe local pain. The grade of pain was estimated using RTOG score (0-9). They were irradiated with conventional fractionation with dose of 60-70 Gy. Hyperthermia was performed within 30 minutes after each irradiation once or twice a week using RF capacitive heating equipment, Thermotron RF-8. In the patients with Pancoast tumor, the average radiation dose was 70 Gy and the average number of heating was 7. Intratumoral temperature over 42°C was obtained in 7/13 of the patients. Relief of pain was observed in 13/13 of patients and the average pain scale decreased from 5.5 to 1.4. Improvement ratio of the pain was greater in these group than that in the patients (historical control) who were treated by radiotherapy only.<BR>In the patients with recurrent rectal or rectosigmoid cancer, the average ratiation dose was 63 Gy and the average number of heating was 9.4. Intratumoral temperature over 42°C was obtained in 10/15 (67%) of the patients. Relief of pain was observed in 14/15 (93%) and average scale decreased from 3.5 to 1.9. Improvement of PS was also observed in both of patients with Pancoast tumor and recurrent rectal or rectosigmoid cancer. Combination therapy of irradiation and hyperthermia is an effective means for controlling pain and improving PS and QOL in the patients with advanced cancers.
