Intra-articular fractures of the distal radius in adults can be classified into three groups according to the mechanism of injury; 1) Barton type produced by shearing force, 2) bursting type produced by axial compression to the radius, and 3) Colles type produced by oblique force transmitted from the palm. Most intra-articular fractures can be classified as Colles type. In this group, the traction force produces a transverse fracture on the volar side. The compression force produces dorsal tilting on the joint surface with split shell fragment resulting on the dorsal side. Further intra-articular comminutions may also occur. If anatomical reduction is necessary, we think it is, then open reduction is recommended as follows. Firstly, the volar displacement is corrected through a transverse skin incision. Secondary, through a dorsal lazy S skin incision, split fragment is removed. Finally, the dorsal tilting should be elevated carefully and the defect caused by reposition packed with chip bone. From a sample of eight cases the prognosis of this operation is favorable.
