Comparison of the Ender Nailing and the Compression Hip Screw Fixation for Intertrochanteric Fractures.

In a series of 75 patients with intertrochanteric fractures of the femur, 51 patients were treated by the compression hip screw (CHS) fixation and 24 patients of them were treated by the Ender nailing.Ender's procedure required less operative time and blood loss than CHS. However, the incidence of failure of fracture fixation was higher, especially with the unstable types of the fracture, in the group treated by the Ender nail. On the other hand, we obtained successful results even with the unstable types in the group treated by the CHS fixation.It is our opinion that the CHS is preferable in both the stable and the unstable intertrochanteric fractures, although the Ender nail can be indicated for elderly patients of poor risk with stable fractures and is also applicable to elderly patients with unstable fractures who do not necessarily have to gain high activity.
