Clinical Study of Migration of Compression Hip Screw for Intertrochanteric Fractures.

Since 1981, the compression hip screw technique was used for 116 patients (35 male, 81 female) with intertrochanteric fracture. Average age of the patient at the time of surgery was 76.7 years old (range, 42-95). Average follow up after surgery was 8.0 months (range, 3-47).In 11 cases, the screw migrated more than 15mm and 6 cases had posterior fragments. Twenty-nine cases had posterior fragments on lateral X-ray view. Twelve cases were treated by setting the anterior cortex in apposition to each other so that the proximal fragment was anterior and migrated 5mm on average, the others migrated 17mm.It is necessary to set the anterior cortex in apposition to each other for unstable intertrochanteric fractures which have posterior fragments.
