Perioperative management for the emergent surgery of the thoracic aortic aneurysm. Experience with 26 patients.

Perioperative management of the twenty six cases of emergent surgical treatment of thoracic aortic aneurysm from November, 1981 to March, 1988 were evaluated. Eighteen patients were men and nine were women. Average age was 65.8 years old. 18 out of 21 patients with aortic dissection were classified (Stanford) into Type A and 3 into Type B. True aneurysm in 5 patients was located Ascending aorta in two, Aortic arch in one, Aortic arch and Descending aorta in one and Descending aorta in one. Emergent surgeries were done in 22 out of 26 patients due to the rupture of aneurysm. The survival rate which was 23% before 1986 increased to 69% after 1987. The reason why the survival rate improved were; 1. Preoperative diagnosis was made immediately by using the non-invasive methods such as computed tomography. 2. Stabilization of hemodynamics and shorter preoperative time was achieved by the anesthesiologists. 3. Sternotomy was performed after the cannulations of femoral artery and vein for the cardiopulmonary bypass, so that the intraoperative death due to bleeding decreased.
