A Case of Cardiac Arrest Following Minitracheotomy

This is a case report of a 66-year-old female patient with multiple myeloma who developed cardiac arrest after a minitracheotomy. The patient was admitted to the intensive care unit for treatment of respiratory failure. Four days after artificial ventilation, her respiratory condition improved sufficiently to allow tracheal extubation. However, a minitracheotomy had to be performed because of sputum retention. Ten minutes after the minitracheotomy, unexpected bleeding from the site of the minitracheotomy increased. Subsequently, oxygen saturation dropped rapidly and then cardiac arrest occurred. She was immediately resuscitated by endotracheal intubation and cardiac massage. Bronchofiberscopic examination revealed that a large amount of sputum with clots had occluded the right main bronchus. We recommend that bronchofiberscopic examination be performed before minitracheotomies to reduce the risk of serious complications occurring due to this tecqunique.
