Macrolides and Airway Defence Mechanism. Macrolide Antibiotics and Inflammatory Cells.

We have recently described the effect of macrolides on the kinetics of inflammatory cells in bronchoalveolar lavage fluid (BALF) or peripheral blood obtained from patients with diffuse panbronchiolitis (DPB). Patients were found to have significant higher percentages of neutrophils, higher levels of neutrophil chemotactic activity (NCA), interleukin (IL) 113, IL-8, leukotriene B, (LTB4) and defensin in BALF before macrolide therapy than healthy volunteers. They showed a significant reduction in all of these factors after long-term macrolide therapy. Mac-1 expression, a neutrophil adhesion molecule was also significantly elevated in peripheral blood from DPB compared with that from healthy volunteers, and a significant reduction in Mac-1 expression was observed after macrolide therapy. Additionally, the number of lymphocytes was significantly increased in BALF before the therapy. BALF also revealed a significant increase in the number of CD44-HLA-DR+ cells and CD8+HLA-DR+ cells. The majority of CD4+I-ILA-DR+ cells and CD8+1-ILA-DR+ cells were memory T cells and cytotoxic T cells, respectively. After macrolide therapy, a significant reduction in the number of lymphocytes and activated CD8+ cells, plus an elevation in the CD4/CD8 ratio in the BALF of DPB patients, was observed. These findings suggest that neutrophils and activated CD8+ cells are important cellular components of bronchial inflammation in DPB.
