Energy expenditure and nitrogen balance were measured in four vegetative patients. Energy expenditure was measured by the indirect calorimetry method early in the mornings (basal metabolic rate; BMR) and in the afternoons (resting metabolic rate; RMR). The results were compared with those of healthy Japanese of the same age and of the same body size. BMR of four subjects significantly decreased by 27% to 48%, but thyroid hormone concentration was in the normal range. Creatinine coefficient as the body muscle mass indicator and 3-methylhistidine/creatinine ratio as the body muscle metabolism indicator were decreased. Body fat percent of three vegetative patients was high. BMR decrease was accompanied by decrease of active tissues due to the long period of the resting state. RMR of all the subjects also decreased but the values were 1.1 times higher than BMR. SDA (specific dynamic action) was observed in all the patients and nitrogen balance values were negative in three of them, but the values did not change by increased protein administration in the two subjects.
